Job summary

Pair Team is seeking a Lead Care Manager to advance our remote-first, community-based care model in California. You’ll engage with individuals facing serious mental illness or homelessness, coordinating care with interdisciplinary teams and community partners.

Expect mostly remote work (≈90%), with 2–3 on-site community visits per month near your city. You’ll build trust, set health goals, and guide members through Health Action Plans, while leveraging CRM tools and telecommunication to stay

Qualifications

1+ years of general work experience (Case Management preferred).
You are physically located in California.
Bilingual – English/Spanish.
Strong understanding of cultural fluency.
High degree of empathy.
Ability to work collaboratively in a multidisciplinary team.
An eye for optimization.
Organizational skills.
Ability to remain patient when faced with adversity.
Strong technical skills with CRM databases, Excel, Word, email, and video conferencing.
A valid driver's license and auto liability insurance.

Responsibilities

Maintain ongoing caseload of individuals through evidence-based approaches to promote engagement and health goal achievement.
Use relationship-based strategies to support members with social support navigation.
Conduct periodic telephonic and SMS outreach for timely follow-ups.
Identify health/wellness goals and incorporate into Health Action Plan/Shared Care Plan.
Support nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with tasks.
Collaborate on care issues with Enhanced Care Management team and consult before clinical actions.
Meet monthly encounter metrics to ensure regulatory compliance.
Identify and break down barriers to program continuation.
Assist individuals in connecting to community supports, scheduling appointments, managing referrals, and follow-ups.
Coordinate physical care management with external and internal providers.
Use external and internal platforms to collaborate with team and carry out daily tasks.
