Job summary

Pair Team is seeking a highly motivated Lead Care Manager to join our mission-driven team. The role focuses on outreach and engagement for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and high medical needs, with primary work from home and 2–3 on-site visits per month in the local community.

The position requires bilingual English/Spanish communication, strong empathy, and collaboration across disciplines.

Qualifications

• 1+ years of general work experience (Case Management preferred)
• Bilingual English/Spanish
• Strong empathy and cultural fluency
• Ability to work collaboratively in a multidisciplinary team
• Strong technical skills with CRM, Excel, Word, email, and video conferencing

Responsibilities

• Maintain ongoing caseload of individuals using evidence-based approaches to promote engagement and health goals
• Use relationship-based strategies to support social supports and navigation for members
• Conduct periodic telephonic and SMS outreach to ensure timely follow-ups
• Work with members to identify health/wellness goals and integrate them into care plans
• Assist nurses, behavioral health managers, and community engagement specialists with delegated tasks
• Collaborate on care issues with Enhanced Care Management team before clinical actions
• Meet monthly encounter metrics to ensure compliance with health plan regulations
• Identify and address barriers to program continuation for individuals
• Assist individuals in securing community supports and arranging referrals
• Coordinate physical care management appointments with providers
• Utilize online platforms to collaborate with team members and manage daily tasks
