Job summary

Providence Seaside Hospital-Seaside seeks a Licensed Clinical Social Worker (LCSW) to provide biopsychosocial assessments, diagnoses and care interventions. The role collaborates with the IDT to develop person-centered care plans across the care continuum for inpatients and outpatients.

The LCSW will work independently within Social Work Services, adhering to NASW ethics and the hospital's mission to reduce disparities and advocate for patients and families.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work or candidacy status by CSWE.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License upon hire.
• License in states wherever care is delivered.
• At least 2 years of relevant Social Work experience (may include CSWA experience).

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy for patients and their support systems.
• Collaborate with patients and support systems to develop evidence-informed person-centered care plans across care continuum with the Inter/Multi-Disciplinary Team.
• Work with inpatient and outpatient populations and uphold NASW ethics and SOP.
