Job summary

Providence Willamette Falls Medical Center is seeking an experienced Licensed Clinical Social Worker (LCSW) to join Social Work Services on a full-time day shift. The LCSW will perform biopsychosocial assessments, provide diagnosis and interventions, and advocate for patients and families across the continuum of care.

The role emphasizes collaboration with an interdisciplinary team, commitment to ethical practice, and addressing health disparities to ensure patient-centered care.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited school or candidacy by CSWE.

Responsibilities

• Provides comprehensive biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions/advocacy for patients and support systems.
• Collaborates with the patient and IDT to develop and implement person-centered care plans across the care continuum.
• Adheres to NASW Code of Ethics and Standards of Practice while addressing health disparities and promoting patient self-determination.
