Job summary

Providence Seaside Hospital in Seaside, Oregon is seeking a Social Worker, LCSW on-call with a variable schedule. The LCSW independently assesses, diagnoses and provides biopsychosocial care, collaborating with the IDT to develop person-centered care plans across the care continuum.

Minimum qualifications include CSWE-accredited MSW, Oregon LCSW license upon hire, licensure in states served, and at least two years of social work experience; acute care experience preferred.

Qualifications

• Master's Degree in Social Work from a CSWE Accredited School of Social Work.
• Oregon Clinical Social Worker License upon hire.
• License in states wherever care is delivered.
• 2 years Relevant Social Work experience; CSWA acceptable.

Responsibilities

• Independently provides biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy.
• Collaborates with patient and support system to develop and implement person-centered care plans with the IDT.
• Works with inpatient or outpatient population, upholding NASW code of ethics and mission.
