Job summary

Rehab Without Walls Neuro Rehabilitation, affiliated with BrightSpring Health Services, seeks a Licensed Clinical Social Worker (LCSW) or LCPC for community-based care. The role focuses on real-world psychosocial support delivered in home and community environments, helping patients and families adapt and rebuild daily function.

You will work with a neuro rehab team that values autonomy, flexibility, and meaningful clinical impact, with an emphasis on flexible scheduling and local travel.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current LCSW licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain or spinal cord injuries or acute diagnoses.
• CPR certification required.
• Ability to lift 50 pounds; may involve up to 100 pounds with safety procedures.
• Moving, lifting, or transferring of patients as needed.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social and emotional factors affecting health.
• Instructs patients and families on coping strategies.
• Participates in care coordination and resource navigation.
• Identifies and connects patients with community resources.
