Rehab Without Walls Neuro Rehabilitation in the Sacramento area is seeking a Licensed Clinical Social Worker to join our home-based, community-focused care team. You will conduct comprehensive evaluations, set patient-centered goals, and collaborate with physicians and clinicians to support recovery.

Flexible scheduling and an hourly compensation model are offered, with travel reimbursement and growth opportunities.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting.
• Current BLS First Aid and CPR certification required.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social/emotional factors.
• Instructs patients and families in coping with social and emotional responses.
• Participates in care coordination and identifies patient needs.
• Connects patients/families with appropriate community resources.
