Qualifications

• Master’s degree in Social Work required.
• 3 years’ healthcare experience preferred.
• Pediatric experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Provide psychosocial assessment, education, and discharge planning to support identified barriers to care.
• Counsel families, offer emotional support, and coordinate with care teams.
• Coordinate with community resources and outside agencies for continuity of care.
• Provide crisis intervention and ongoing counseling for patients and families.
• Maintain documentation in the medical record and participate in team and family conferences.
• Travel to ambulatory clinics and hospital campus for education and meetings.
