Job summary

Rehab Without Walls Neuro Rehabilitation is hiring licensed clinical social workers in the Greater Sacramento area to provide counseling and support in patients' homes and communities. You will work with individuals to address physical, mental, cognitive, and emotional adjustments through recovery, offering interventions and resource referrals to meet personal and independence goals.

Qualifications

• Must hold current Licensed Clinical Social Worker Licensure in the practicing state.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis.
• BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; may involve lifting up to 100 pounds with safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.

Responsibilities

• Performs comprehensive evaluations of patients and families and their social/emotional needs.
• Sets measurable objectives with rehab team.
• Assists physicians and health team in understanding social and emotional factors affecting health problems.
• Instructs and counsels patients and families in coping with social and emotional responses to illness.
• Participates in care coordination and serves as a resource to health team members.
• Identifies and helps patients/families access appropriate community resources to meet objectives.
