Qualifications

Responsibilities

• Collaborates with interdisciplinary teams to coordinate patient care and resolve care transition issues.
• Develops discharge plans in coordination with the Care Manager and coordinates post-hospital placement and social services.
• Refers cases to physicians or RNs when criteria are not met to ensure timely interventions.
• Acts as liaison with community agencies to facilitate seamless transitions for discharged patients.
• Facilitates interdisciplinary meetings to address patient care needs and support planning.
• Maintains accurate documentation of case management activities and compliance with reporting requirements.
• Ensures compliance with first-person reporting and federal mandates when applicable.
