Job summary

CHS Career Site is seeking a Licensed Medical Social Worker - NICU at North Knoxville Medical Center in Powell, TN. The Care Manager will coordinate discharge planning, transitions of care, and case management to ensure optimal patient outcomes.

Requires LCSW licensure (LMSW acceptable) with 2–4 years in acute care; Bachelor's in Social Work is required, MSW preferred. This is a part-time, day-shift role emphasizing collaboration with IDT and compliance with standards.

Qualifications

• Bachelor's degree in Social Work required.
• Master's degree in Social Work preferred.
• 2–4 years of experience in an acute care hospital setting required.

Responsibilities

• Collaborates with interdisciplinary teams to coordinate patient care and ensure timely transitions.
• Develops and supports discharge plans with post-hospital placement and social services.
• Refers cases to physicians, managers, or RNs when criteria are not met, ensuring timely interventions.
• Serves as liaison with community agencies to facilitate seamless patient discharges.
• Maintains accurate documentation of case management activities and compliance with reporting requirements.
