Tennova Healthcare- LaFollette Medical Center is seeking a Care Manager - RN to coordinate and oversee discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. This role collaborates with interdisciplinary teams, reviews medical records for appropriateness and medical necessity, and maintains compliance with federal, state, and accreditation standards.

• Active RN license in state of employment
• 2–4 years of clinical nursing experience in hospital, home health, or nursing home setting
• 2–4 years of care management experience preferred
• ACCM (Accredited Case Manager) or CCM preferred

Responsabilidades

• Conduct daily reviews of medical records to assess admission appropriateness and utilization of services.
• Collaborate with interdisciplinary teams to ensure effective communication and care coordination.
• Develop and implement discharge plans, coordinating post-hospital placement and social services.
• Refer cases to physicians or managers when criteria are not met for timely interventions.
• Serve as liaison with community agencies to facilitate smooth transitions for discharged patients.
• Lead interdisciplinary meetings to address patient care needs and care planning.
