BlueCross BlueShield of Tennessee, Inc. is seeking a Care Transition Case Manager to support members transitioning from hospital to home and community care. The role emphasizes safe discharge planning, coordination with hospital care teams, and post-acute services to improve health outcomes.

Expect in-person visits and collaboration with facility staff within Knoxville-area hospitals. Ideal candidates have RN licensure in TN (or compact state), 3–5 years in health care, and experience in home

Formación

• RN license active in TN or state in Nurse Licensure Compact.
• 3+ years clinical experience in healthcare required.
• 5 years experience in health care industry recommended.

Responsabilidades

• Support utilization management for complex cases as needed.
• Liaise between members, providers and internal/external customers for care delivery and benefits.
• Oversee highly complex cases to ensure effective interventions and efficient utilization of benefits; perform core case management: assessment, planning, implementation, coordination, monitoring, outcomes, evaluation.
• Digital positions require effective communication via digital channels and provide technical support.
• Position requires 18-month commitment before posting to other internal roles (as of 7/22/2013).
• Immunizations or medical tests may be required.
• Job requires digital literacy assessment.
