Job summary

Rehab Without Walls Neuro Rehabilitation seeks an LCSW in Corpus Christi, TX to provide 1:1 patient care and contribute to social work support within a neurorehabilitation program that extends into home and community settings.

Join a collaborative, autonomous team delivering goal-driven care, expanding neuro rehab expertise in adult populations and coordinating care across disciplines.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) is required.
• Current Licensed Clinical Social Worker Licensure in the state of practice is required.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain injury, spinal cord injury, or acute diagnosis is required.
• Current BLS First Aid and CPR certification is required.
• Ability to lift 50 pounds; may involve lifting up to 100 pounds.
• Duties require fine motor skills, visual acuity, and standing/walking for extended periods.
• Physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and frequent positioning.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with the rehab team.
• Assists health team in understanding social/emotional factors affecting health.
• Instructs/counsels patients and families on coping strategies.
• Participates in care coordination and resource identification.
• Helps patients/families access community resources to meet goals.

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

What you'll need:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
