Job summary

Rehab Without Walls Neuro Rehabilitation seeks an experienced Licensed Clinical Social Worker (LCSW) to support patients in home and community settings. You will perform evaluations, set rehab goals with the team, and assist families in navigating social and emotional aspects of recovery.

You'll provide counseling, coordinate care, and help connect patients to community resources. A Master's in Social Work and current LCSW licensure are required, with BLS/CPR certification preferred and physical

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current LCSW licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with brain or spinal injuries or acute diagnoses.
• BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; moving/handling up to 100 pounds.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and frequent positioning.

Responsibilities

• Performs comprehensive evaluations, assessing social, emotional and support needs of patients and families.
• Sets measurable objectives in collaboration with the rehab team.
• Assists physician and health team in understanding social/emotional factors related to health problems.
• Instructs/counsels patients and families in coping with social and emotional responses to illnesses.
• Participates in care coordination and acts as a resource to health team.
• Identifies and helps patients/families utilize community resources to meet objectives.

Responsibilities

• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

What you'll need:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
