Rehab Without Walls Neuro Rehabilitation in Austin, TX is seeking a licensed clinical social worker (LCSW) to join our Home & Community Rehabilitation team. You will evaluate social and emotional needs, collaborate with the rehab team, and help patients and families access community resources.

The role emphasizes compassionate counseling, care coordination, and support for neurological injuries such as stroke or TBI, with a flexible schedule and no weekend requirements.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) and active licensure in the practicing state.
• Minimum of one year experience in a rehabilitation setting (neuro/trauma).
• BLS First Aid and CPR certification current and valid.

Responsibilities

• Perform comprehensive evaluations of social, emotional, and support needs for patients and families.
• Set measurable objectives with the rehab team to guide care planning.
• Assist physician and health team in understanding social and emotional factors affecting health.
• Counsel patients and families in coping with illness and related emotional responses.
• Participate in care coordination and act as a resource to health team members.
• Identify and connect patients/families with appropriate community resources.

Rehab Without Walls is hiring a Licensed Clinical Social Worker (LCSW) to join our Home & Community Rehabilitation team in Austin,TX!

Who we are looking for:

• We're seeking an experienced LCSW to support individuals with neurological injuries such as stroke and traumatic brain injury (TBI)
• In this vital role, you'll provide therapeutic interventions to promote emotional and social adjustment, and community resource navigation
• Prefer 2+ years of experience with knowledge of neurological injuries and/or medical social work background

Responsibilities

What you will do: (responsibilities listed are included but not limited to)

• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
• Sets measurable objectives that are formulated in conjunction with the rehab team
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives

Qualifications

What you'll need:

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• Current BLS (Basic Life Support) First Aid and CPR certification required
• Ability to lift 50 pounds
• Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
• Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
