Job summary

PATH is seeking an Outreach Case Manager to join the Metro Rail Outreach Team in Los Angeles, CA. The role conducts daily street outreach to connect people experiencing homelessness with housing, health and supportive services, reporting to the Metro Rail Outreach Team.

You will develop ISP plans, coordinate referrals, and advocate for clients with landlords, with driving required and a full-time schedule. Minimum qualifications include 2 years in outreach case management, valid eligibility,

Qualifications

• Two (2) years of experience in outreach case management or a related field.
• Must have employment eligibility verification.
• Must obtain CPR/First Aid training or be willing to do so.
• Pass tuberculosis test, background screening and drug test as a hire condition.
• Driving is an essential function; reliable transport and valid license required.
• California-based work expectations; no fully remote positions.

Responsibilities

• Daily street outreach to develop rapport with clients and assist them in finding housing.
• Respond to outreach requests within contract timeframes and provide follow-up information.
• Conduct VI-SPDAT assessments and connect clients with appropriate interventions.
• Develop ISP with clients to address short- and long-term goals.
• Provide street-based case management and help link to long-term services.
• Meet with clients regularly to review progress and support goals.
• Assist with documentation, housing referrals, and benefits applications.
• Advocate for clients with landlords and coordinate housing navigation.
