Corewell Health in Royal Oak is seeking an Outreach Transplant Nurse Clinician to develop, monitor and execute outreach initiatives for the Kidney Transplant Program. You will educate ESRD patients, coordinate with nephrologists and dialysis units, and partner with marketing to create educational materials.

You will collect and maintain quality data on referrals and transplants, support waitlist management, and collaborate with transplant teams to optimize patient care across multiple sites.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• 3 years general RN experience; 2 years in transplant, nephrology or dialysis preferred.
• Proficient in Word, Excel, PowerPoint; strong problem solving and communication.
• Must communicate effectively and collaborate with a multi-disciplinary team.

Responsabilités

• Educates ESRD patients and families about kidney transplantation as a treatment option.
• Develops and maintains outreach programs to foster relationships with referring physicians and dialysis staff.
• Coordinates outreach events with transplant surgeons/nephrologists to grow the program.
• Works with marketing to develop educational and promotional materials.
• Tracks referrals, evaluations, candidate listings and transplants; maintains quality data.
• Maintains multiple databases for referral tracking and statistics.
• Collaborates with the Transplant Program Coordinator for referral triage.
• Supports pre‑transplant nurse coordinators with clinical information updates.
