Job summary

Northwestern Medicine is seeking a Transplant Nurse Coordinator to serve as the primary coordinator for deceased donor transplant activity with 24/7 on-call responsibility. You will manage organ offers, donor logistics, and multi-disciplinary communication, while coordinating patient care across all phases under transplant physicians’ supervision.

The role requires a BSN, IL RN license, and CPR-BLS, with 2–3 years of related clinical experience.

Qualifications

• RN license in Illinois required.
• CPR-BLS certification required.
• Minimum 2–3 years of related clinical experience.

Responsibilities

• Coordinate deceased donor transplant activity from notification to transplantation.
• Manage donor organ offers and transplant logistics during on-call shifts.
• Communicate with multidisciplinary team and external clinicians.
• Coordinate care planning, data collection, referrals, and follow-up visits.
• Educate patients, families, and staff on transplant processes.
• Ensure compliance with policies, procedures, and regulatory standards.
