Qualifications

• ADN required for RN licensure in Illinois.
• Illinois RN license required/current.
• CPR/BLS certification required.
• 1-2 years nursing experience relevant to transplant care.
• Strong clinical assessment, care coordination and patient education skills.
• Excellent interpersonal and communication abilities.
• Ability to manage multiple priorities in a fast-paced transplant setting.
• Strong organizational skills and attention to detail.

Responsibilities

• Coordinate clinical care of kidney transplant patients across pre- and post-transplant care.
• Facilitate transplant evaluations with multidisciplinary team.
• Review labs and diagnostics to support evaluations and MAB discussions.
• Serve as primary contact for transplant patients and families.
• Educate patients and caregivers on procedures, medications, and follow-up care.
• Conduct telephone triage and coordinate timely interventions.
• Maintain accurate EMR documentation.
• Discuss consent forms and treatment plans with patients and families.
• Ensure accurate UNOS waitlist management and communications.
• Prepare/submission of information to UNOS, CMS, Joint Commission and insurers.
• Participate in quality improvement and outcomes reporting.
• Collaborate with teams to improve patient care and transplant processes.
• Attend departmental meetings and committee activities.
• Provide cross-functional support across transplant program operations.

Education

Associate Degree in Nursing (ADN)

Bachelor's Degree in Nursing (BSN)

Illinois RN License

CPR/BLS Certification

Nursing experience 1-2 years
