Qualifications

• ADN with Illinois RN license and CPR/BLS certification.
• 1-2 years of relevant nursing experience.
• Strong clinical assessment, care coordination and patient education.
• Excellent interpersonal and communication abilities.
• Ability to manage multiple priorities in a fast-paced clinical environment.

Responsibilities

• Coordinate clinical care of kidney transplant patients across pre- and post-transplant stages.
• Facilitate transplant evaluations with the multidisciplinary team.
• Review labs and diagnostics to support patient evaluations.
• Serve as primary contact for patients and families, offering guidance and care coordination.
• Educate patients on procedures, medications, follow-up care, nutrition and wellness.
• Maintain accurate EMR documentation and consent discussions.
• Ensure waitlist management and UNOS communications; participate in regulatory reporting.
• Engage in quality improvement initiatives and outcomes monitoring.
