Job summary

WC Health, a Behavioral Health Company, seeks a dedicated Junior Case Manager to support Medicaid members in our housing program. You will assist with needs assessments, treatment plan coordination, and care coordination.

The role requires strong communication, trauma‑informed, and person‑centered services. You will travel between two housing locations and collaborate with an interdisciplinary team. Pay is $17/hr.

Qualifications

• Education: High School Diploma or equivalent required and QMHA eligibility.
• Bachelor's degree in Human Services, Psychology, Social Work, or related field required.
• Experience with Medicaid members or Medicaid-funded behavioral health services preferred.
• Knowledge of HIPAA and patient confidentiality; strong communication skills.

Responsibilities

• Assist Primary Case Managers with monitoring and management of treatment plans and care needs.
• Coordinate with housing program staff and clinicians; document interactions in EHR.
• Conduct Needs Assessments via virtual or in‑person meetings; coordinate transportation and access to services.
