Qualifications

Through the HUD-funded Jobs Plus Program, this position plays a vital role in helping residents overcome barriers to employment, connect with critical resources, and create pathways toward sustainable careers and improved quality of life.If you are driven by a desire to help others succeed and thrive in a collaborative, mission-driven environment, we encourage you to apply.PurposeThe Jobs Plus Case Manager provides comprehensive case management, employment support, and service coordination for participants in the Jobs Plus Program. This position works directly with residents to identify barriers to employment, develop individualized employment and self-sufficiency plans, connect participants with supportive resources, and provide ongoing coaching and support throughout their employment journey. The role is responsible for recruiting program participants, conducting assessments, maintaining case records, and collaborating with community partners to improve outcomes for residents.Key ResponsibilitiesCase Management & Participant SupportConduct eligibility screenings, assessments, and intake activities for program participants.Develop individualized employment plans and goal-oriented service plans.Provide intensive case management and ongoing support to help participants achieve employment and personal goals.Monitor participant progress and adjust service strategies as needed.Respond to participant crises and connect individuals with appropriate resources and support services.Employment & Resource CoordinationConnect participants with job training, workforce development opportunities, and employment resources.Identify and address barriers to employment, including childcare, transportation, legal services, substance abuse treatment, counseling, and other supportive services.Provide coaching and guidance related to budgeting, life skills, stress management, and workplace success.Support participants during job placement and the early stages of employment to promote retention and success.Outreach & Community EngagementConduct outreach activities, home visits, and recruitment efforts within the community.Collaborate with employers, community organizations, service providers, and program partners to expand opportunities for participants.Assist with the development and facilitation of Job Clubs, support groups, workshops, and mentoring activities.Promote awareness of program services and encourage resident participation.Program AdministrationMaintain accurate case files, participant records, and program documentation.Assist with data collection, reporting, and grant compliance requirements.Monitor participant outcomes and program performance metrics.Support continuous improvement efforts and program initiatives.Supervise support staff and consultant contracts as assigned.a { text-decoration: none; color: #464feb;}tr th, tr td { border: 1px solid #e6e6e6;}tr th { background-color: #f5f5f5;}Required Education & ExperienceBachelor's degree in Social Work, Sociology, Human Services, or a related field required.Minimum of two (2) years of experience in case management, service coordination, workforce development, community outreach, social services, or a related field.Experience working with underserved populations and individuals facing barriers to employment preferred.An equivalent combination of education and relevant experience may be considered.a { text-decoration: none; color: #464feb;}tr th, tr td { border: 1px solid #e6e6e6;}tr th { background-color: #f5f5f5;}Required Skills and AbilitiesProfessional SkillsStrong case management, assessment, and service coordination skills.Ability to develop individualized plans and guide clients toward successful outcomes.Knowledge of community resources, workforce development programs, and supportive services.Ability to manage sensitive information with professionalism and confidentiality.Communication & Relationship BuildingExcellent interpersonal, verbal, and written communication skills.Ability to establish trust and build positive relationships with residents, employers, partners, and community organizations.Strong customer service skills and a commitment to helping others succeed.Ability to work collaboratively across multiple departments and organizations.Organizational & Analytical SkillsStrong organizational skills with exceptional attention to detail.Ability to manage multiple priorities in a fast-paced environment.Strong problem-solving and decision-making abilities.Ability to work independently while maintaining accountability for results.Technical SkillsProficiency with Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook.Ability to maintain electronic case files, prepare reports, and work with databases and case management systems.Ability to learn and utilize additional software applications as necessary.NOTE: a { text-decoration: none; color: #464feb;}tr th, tr td { border: 1px solid #e6e6e6;}tr th { background-color: #f5f5f5;}We would like to make you aware that this position is grant-funded and is funded through May 2027.
