Position Summary
Coaches and supports individuals served in community employment by teaching job tasks/skills and encouraging independent work skills. Assists individuals with learning bus routes to and from work as well as provide support in learning other modes of travel to work. Assesses work strengths and weaknesses andidentifiesandimplementsappropriate jobaccommodations.
Job Responsibilities
- Coachesindividuals served at community job sites by providing orientation and training,assistingwith job relatedissuesanddemonstratingjob functions.
- Utilize training techniques through learning channels such as verbal prompting, visual prompting, forward chaining, etc. Adaptstraining techniques based uponparticipantsstrengths and needs.
- Encourages andassistsindividuals served by teaching job tasks, interpersonal skills, stress management, communication skills, and otherjob-relatedactivities.
- Providessupportand guidanceto individuals completing acommunity-basedwork assessment. Identifywork skills, strengths, interests, and make recommendations for a realistic vocational direction for the individual served.
- Identifiesandassistsemployers and individuals served with implementing any needed job modification.
- Provides problem solving, mediation, and advocacy by teachingindividuals servedto communicate effectively and appropriately with supervisors and co-workers.
- Serves asliaisonfor individuals served withemployer, case managers, fundingagency, and others byfacilitatingcommunication.
- Provides transportation by driving individuals served to and from work sites for orientation, training,community-basedworkassessmentsand other activities as needed.
- Travel trains individuals served to and from work sites.
- Keepsaccuraterecords by completing andmaintainingrequired documentation including case notes, monthly summaries and reports, intake forms, activity logs, mileagereportsand discharge summaries.
- Provides coverage at other agency training programs as needed.
- Maintains knowledge and skills by attending meetings, seminars,trainings, and workshops.
- Maintains confidentiality of individual case records and other sensitive subject matter.
- Provides follow alongsupportstoindividualsserved by performing site visits and contacting employers by phone.
- Communicates regularly with Easterseals Redwood supervisor and managerregardingprogress ofindividual served.
- Works assigned scheduleandexhibitsregular and predictable attendance
- Works early mornings, evenings, and weekends onoccasionto meet workload demands.
Education/Qualifications:
- High schooldiploma. Somecollegepreferred.
- Experience working with individuals with disabilities preferred.
