Job summary

Easterseals Redwood is seeking a Service Coordinator to develop, implement, and monitor annual service plans for individuals with disabilities in Kentucky. The role collaborates with employers, staff, and support personnel to resolve issues and ensure successful employment outcomes.

The position emphasizes communication, advocacy, and coordination across teams to deliver person-centered supports and access to community resources. A bachelor's degree is required.

Qualifications

• Bachelor's degree from an accredited college or university.
• One year of experience in developmental disabilities OR equivalent experience/credentialing.

Responsibilities

• Develop, implement and monitor annual individual service plans.
• Assist individuals with disabilities in obtaining or maintaining a job and resolve issues with employers and staff.
• Attend job development and coaching sessions with the individual served.
• Ensure clear communication between individuals and supervisors.
• Make referrals for funding support and provide direct service as needed.
• Orient new individuals to the service location and monitor progress during 30-day assessments.

Position Summary:

The Service Coordinator will be responsible for developing, implementing and monitoring annual individual service
plans.

Job Responsibilities:

• Assisting individuals with disabilities in obtaining, learning or maintaining a job by identifying problem areas and
• working with employers, Easterseals Redwood staff and other support personnel to resolve any issues
• Attends job development and job coaching sessions with the individual served.
• Ensures clear, effective communication has taken place between individuals and supervisors by regularly visiting
• Makes referrals to funding agency (i.e., OOD) for new or different jobs and advocates for the individual for
• funding support.
• Provides direct service if staffing levels are below the amount specified by acuity as directed by supervisor.
• Meets with new individuals to orient them to the service location.
• Monitors progress during 30-day assessment, coordinates staffing, reviews progress with individual and support
• team, and administers vocational interest testing.

Education/Qualifications:

• Minimum of a baccalaureate degree from an accredited college or university and one year of experience in the
• field of developmental disabilities;
• Or Relevant experience on a year-for-year basis, or credentialing that substitutes for the educational requirement;
