Job summary

Active Day is seeking an ODP Program Specialist to support adults with intellectual disabilities by developing individualized goals aligned with ISP plans. You will participate in ISP meetings, assess progress, and ensure regulatory compliance through admission processes and ongoing documentation.

The role emphasizes detailed organization, strong communication, and collaboration with Center Directors to deliver compliant, person-centered care.

Qualifications

• Associate degree/60 credits plus 4 years' work with persons with disabilities
• Bachelor's degree plus 2 years' work with persons with disabilities
• Master's degree plus 1 year' work with persons with disabilities
• Minimum 1 year experience working with adults with intellectual disabilities preferred
• Must be extremely detail-oriented with exceptional organizational skills to maintain compliance
• Strong communication and relationship-building skills
• Ability to multi-task and manage competing priorities
• Working knowledge of Microsoft Office (Outlook, Word and Excel)
• Background check (and any state-specific requirements) must be satisfactory

Responsibilities

• Coordinate and complete assessments and develops member goals
• Assess member progress with goals and objectives set forth in assessments
• Participate in the development of member ISPs, including any required updates/revisions
• Act as member “Plan” lead relating to ISP development, annual update and revision
• Attend ISP meetings
• Report discrepancies in the ISP and/or changes related to the member’s needs
• Ensure services provided are in accordance with ISP
• Review, sign and date monthly documentation and make recommendations
• Develop and implement provider services with Center Director
• Coordinate training of teammates on health and safety needs
• Perform required documentation to ensure compliance with state regulations
• Support Center staff and members daily and communicate with support teams
• Maintain filing system; prepare case notes and reports
• Attend meetings to address service concerns and enrollment
• Support members with personal care services and mealtime support
• Intervene in crises and maintain proficiency in client techniques
• Maintain licenses and progressions in the field

Job Summary

The Program Specialist provides initial, quarterly and annual assessments for adults with intellectual disabilities. The Program Specialist participates in member Individual Service Plan meetings (“ISP”). The Program Specialist is responsible for ensuring regulatory compliance through the admission process and ensures that all work complies with company policies and procedures, safety and regulatory laws and standards.

Qualifications

• A program specialist shall have one of the following groups of qualifications:
• An associate's degree or 60 credit hours from an accredited college or university and 4 yearsof work experience working directly with persons with disabilities. OR
• A bachelor's degree from an accredited college or university and 2 years of work experienceworking directly with persons with disabilities. OR
• A master's degree or above from an accredited college or university and 1 year of workexperience working directly with persons with disabilities.
• Minimum one (1) year experience working with adults with intellectual disabilities preferred
• Must be extremely detail-oriented with exceptional organizational skills in order to maintaincompliance.
• Strong communication and relationship building skills.
• Ability to multi-task and manage competing priorities.
• Working knowledge of Microsoft Office (Outlook, Word and Excel).
• Successful results of background check (and successful MVR results for Hamilton OH centers only)and any other state-specific requirements.

• Ensure services provided are in accordance with ISP
• Review, sign and date monthly documentation of member participation and progress and makerecommendations if revisions to a service or outcome are needed.
• Develop and implement provider services in partnership with Center Director, as required.
• Coordinate training of teammates in the content of health and safety needs relevant to each member.
• Perform all required documentation to ensure compliance with state regulations.
• Support all Center staff and members daily ensuring needs are met and making necessarycommunication efforts with support teams.
• Maintain a filing system; prepares case notes, reports, summaries, and documentation at therequest of the Center Director.
• Attend meetings to address service concerns, behavioral issues and new member enrollment.
• Supports members as needed with personal care services, mealtime support and other directservice needs.
• Intervene in crises; maintains proficiency in various client techniques (i.e. AEGIS or other statespecific techniques) and represents a positive behavior that supports Active Day’s culture andphilosophy.
• Maintain required licenses and/or certificates; develops and demonstrates knowledge ofprogressions in field in order to provide the most beneficial member care.
