Job summary

Center for Disability Services, Inc. is seeking a Day Program Plan Coordinator to support program management in ensuring ISP Plans, Staff Action Plans and related documentation are completed on time.

The role focuses on developing individualized, person-centered programs to promote independence, community inclusion, productivity, and self-determination. The DPC will implement plans that honor personal choices and the ability to take informed risks, while providing necessary training in First

Qualifications

• Education required: High School diploma or equivalent. Specialized training and certification in First Aid, CPR, SCIP-R and BIPP.
• Training provided by CFDS; some programs may require Medication Administration Certification.
• NYS Driving License in good standing may be required; Road Test by agency personnel.

Responsibilities

• Assist program management to ensure ISP Plans, Staff Action Plans and all corresponding documentation and timelines are met.
• Develop individualized and person-centered program plans to promote independence, community inclusion, personal choice and productivity.
• Ensure plans respect lifestyle decisions and abilities to take informed risks.
