Job summary

Nantucket Cottage Hospital on Nantucket Island seeks a Pharmacist to join our hospital-based team. The role emphasizes sterile compounding, drug information, order review, and collaboration with the medical staff to ensure optimal patient care.

The on-site position includes day shift with on-call rotations. Required credentials include a PharmD/BS in Pharmacy, MA license, and prior hospital pharmacy experience; residency may substitute for experience.

Qualifications

• Bachelor’s Degree in Pharmacy required.
• Current, valid Massachusetts Board of Registration in Pharmacy License required.
• Prior Licensed RPh experience of at least 2 years OR 1-year residency preferred.
• Hospital pharmacy experience strongly preferred.

Responsibilities

• Perform sterile compounding (Hazardous and non-Hazardous) per USP <797>, <800> and Mass BORP.
• Review and dispense medications per department policies.
• Evaluate orders for strength, dose, administration and timing.
• Resolve drug interactions and non-formulary issues.
• Conduct medication reconciliation and address profiles promptly.
• Educate patients and coordinate with nursing/medical teams.
• Manage controlled substances with documentation and audits.
