Job summary

Nantucket Cottage Hospital seeks a Hospital Pharmacist to optimize drug therapy, identify and prevent drug-related problems, and improve therapeutic outcomes under state and federal regulations. May prescribe under CDTM agreements.

Sign-on bonus of $15,000 and relocation/housing stipend of $5,000+ are offered to support relocation to Nantucket. The role emphasizes quality improvement, patient-centered care, and adherence to regulatory standards.

Qualifications

• Doctor of Pharmacy degree required.
• Massachusetts Pharmacist license and MCSR registration preferred; CDTM experience a plus.
• Experience as a pharmacist 2–3 years preferred; strong knowledge of IV admixtures, drug information, ambulatory, manufacturing, and distribution.

Responsibilities

• Directs and coordinates the activities and delivery of pharmacy services.
• Supervises and verifies the work of support personnel during shifts.
• Operates within the limits of pharmacy policy and procedures.
• Ensures compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs.
• Documents and reports clinical interventions, medication errors, adverse drug events.
• Participates in department quality improvement efforts.
• Follows all policies, procedures, laws, and regulations set forth by the Board of Pharmacy and other applicable regulatory agencies.
