Lutheran Social Services of Wisconsin & Upper Michigan is seeking an IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program. This role combines case management, care coordination, social work, disability services, long-term care, and community-based support to help participants live independently.

Formación

• Bachelor's degree requirement or equivalent experience as listed in the ad.
• Four (4) or more years of direct experience with older adults or individuals with disabilities.
• Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption.

Responsabilidades

• Develop and maintain trusted relationships with IRIS participants.
• Assist participants in creating and updating individualized IRIS Support and Services Plans.
• Conduct monthly participant contacts and required quarterly in-person visits.
• Promote participant choice, self-direction, independence, and community inclusion.
• Monitor participant health, safety, and program outcomes.
• Educate participants about IRIS enrollment requirements and resources.
• Collaborate with ADRCs, healthcare providers, and service providers.
• Maintain accurate case documentation and ensure compliance timelines.
