Job summary

PAM Health Rehabilitation Hospital of Tavares is seeking a Patient Assessment Coordinator to manage Medicare IRF/PAI data collection and ensure documentation accuracy. This role requires coordinating data from patient records and maintaining CMS compliance.

The coordinator will also educate staff on CMS guidelines, monitor FIM scoring, and input data into the eRehabData system to support quality reporting programs.

Qualifications

• Graduate of an accredited OT, PT, SLP, or RN program with current state licensure.
• BLS/CPR certified.
• 3–5 years in a patient assessment coordinator role or equivalent.
• ASN required; Bachelor’s degree preferred for nursing; Master’s for OT/PT/SLP.

Responsibilities

• Coordinate data collection for IRF/PAI in the Medicare CMS reporting program.
• Oversee all aspects of IRF/PAI data for accuracy and timeliness.
• Monitor CMS scoring (FIM) and patient care needs implications.
• Input IRF/PAI data into the eRehabData system and ensure supporting docs.
• Educate staff on CMS guidelines and compliance requirements.

Education
