The Joseph H. Neal Health Collaborative seeks an Intimate Health Outreach Coordinator to support community-based health initiatives, coordinating outreach events, testing, partnerships, patient engagement, and linkage to care while ensuring confidentiality.

This role bridges JHN with the community, highlighting access to confidential, respectful health services and a focus on PrEP, HIV prevention, and safer-sex practices. Some travel to outreach locations may be required.

Qualifications

  • High school diploma or GED required; college education or relevant community-health experience preferred.
  • Experience in community outreach, public health, healthcare, HIV/STI prevention, social services, or a related field preferred.
  • Comfortable discussing sexual health, HIV, STIs, PrEP, Hepatitis C, and other sensitive health topics.
  • Strong communication and interpersonal skills.
  • Ability to work respectfully with individuals from diverse backgrounds and communities.
  • Ability to maintain strict confidentiality.
  • Reliable transportation and willingness to travel to outreach locations.
  • Ability to work occasional evenings and weekends based on community events.
  • Willingness to complete JHN-required HIV, STI, Hepatitis C, testing, counseling, and other applicable training.

Responsibilities

  • Coordinate community outreach and Intimate Health screening events.
  • Identify new locations and organizations for JHN outreach activities.
  • Build relationships with community organizations, pharmacies, churches, colleges, recovery programs, and other partners.
  • Promote JHN’s free and low-cost health services throughout the community.
  • Assist with HIV, Hepatitis C, and STI screening activities within the employee's training and certification.
  • Educate community members about PrEP, Doxy-PEP, STI prevention, HIV prevention, Hepatitis C treatment, and safer-sex practices.
  • Connect individuals with JHN clinical services, treatment, medication access, and follow-up care.
  • Assist individuals who receive reactive or positive screening results with appropriate linkage to care.
  • Conduct follow-up outreach to individuals referred to JHN programs.
  • Coordinate outreach calendars, community events, testing schedules, and staffing needs.
  • Maintain adequate outreach and testing supplies.
  • Assist with distributing condoms, educational materials, harm-reduction resources, and JHN promotional materials.
  • Collect and maintain accurate outreach data, including screenings, referrals, encounters, and linkage outcomes.
  • Enter required information into JHN-approved tracking systems.
  • Protect patient confidentiality and follow HIPAA requirements at all times.
  • Represent JHN professionally at community meetings, events, health fairs, and partner locations.
  • Assist with social media and grassroots promotion of upcoming outreach activities.
  • Participate in JHN staff meetings, trainings, and outreach planning sessions.
  • Perform other outreach and program-related duties as assigned.

Position Summary

The Intimate Health Outreach Coordinator supports JHN’s community-based sexual and intimate health initiatives by coordinating outreach, testing events, community partnerships, patient engagement, and linkage to care.
