Joseph P. Addabbo Family Health Center, Inc. is seeking a Prevention Educator to support the Primary Care HIV Prevention program in Arverne, NY.

The role focuses on HIV education, testing, and linkages to care, engaging patients and community members through workshops, outreach events, and on-site services. The position involves collaboration with a diverse community, development of educational materials, and participation in harm reduction strategies to promote PrEP/PEP and sexual health

Qualifications

• Associate’s or Bachelor’s degree in Health Education, Public Health, Social Work, or related field preferred; in lieu of degree, two years of work/volunteer patient contact experience.
• Members of the LGBTQIA+ community are strongly encouraged to apply.
• Driver’s license and personal vehicle preferred; use public transportation if no vehicle.
• At least 2 years of HIV/AIDS prevention, outreach, and health promotion experience preferred.
• Experience with PLWHA, LGBTQIA+, and communities of color preferred.
• Computer literacy with MS Word, Excel, and PowerPoint as well as EMR proficiency.
• Demonstrated organizational, interpersonal, oral, and written communication skills; ability to handle multiple assignments.
• Comfort working in a hybrid (in-person/remote) environment.

Responsibilities

• Engage patients and community in HIV education, HIV screening, counseling, and sexual health promotion.
• Conduct point-of-care visits to discuss HIV testing and prevention options (PrEP/PEP) and condom usage.
• Identify and conduct targeted outreach and recruitment in the community and online.
• Establish partnerships with local schools, places of worship, NYCHA, and community organizations.
• Build relationships with barbershops, hair salons, nail salons, and other local businesses for outreach.
• Perform HIV rapid testing and connect clients to care services.
• Engage individuals in harm reduction strategies including PrEP/PEP and education.
• Review patient charts and registries to identify testing and counseling needs.
• Assist in developing educational materials for patients and staff, including social media posts and newsletters.
• Develop and execute HIV awareness events and health-related education activities.
• Prepare and present educational materials using harm reduction approaches at events.
• Collaborate with providers to reduce barriers to HIV care.
• Maintain accurate patient records of contacts, counseling, testing, referrals, and outreach performance.
• Participate in staff meetings and trainings.
• Assist with special projects and other duties as assigned.

Position Summary

Primary Care HIV Prevention (PCHP) is a U.S. Health Resources and Services Administration funded program aimed at decreasing the risk of HIV transmission, focusing on supporting access to and use of pre-exposure prophylaxis (PrEP). The PCHP program focuses on increasing diagnosis, treatment, and prevention of HIV at JPAFHC and in the communities served by JPAFHC. The PCHP Prevention Educator will work closely with current JPAFHC patients and staff as well as the communities served by JPA to provide HIV education and health promotion services, outreach, HIV testing on- and off-site, and linkages to care. They will play an integral role on the team to counsel, test, and refer at-risk communities to vital HIV, sexual health, and other primary care services at JPAFHC. They will also work closely with the Outreach Coordinator to provide testing, education, and referrals at outreach and partner events in the community.

• 1. Engage JPA patients and community in health education workshops, HIV screening, counseling, and sexual health promotions
• 2. Conduct point-of-care room visits with patients to discuss comprehensive prevention services including HIV testing, PrEP, PEP, and condom usage
• 3. Identify and conduct targeted outreach and recruitment, both traditional and web-based, including but not limited to educational seminars, testing events, and tabling within program’s service areas
• 4. Establish and maintain partnerships at local schools, places of worship, NYCHA, and other community organizations
• 5. Build relationships with non-traditional partners such as barbershops, hair salons, nail salons, and other businesses in the community to provide education and outreach
• 6. Perform HIV rapid testing, delivering results with empathy and ensure clients are connected rapidly to supportive services using the status neutral approach
• 7. Engage individuals in harm reduction strategies including PrEP/PEP, condom distribution, and patient education
• 8. Conduct chart reviews and registry reviews to identify patients that need HIV testing, sexual health counseling, or are PrEP candidates
• 9. Assist in the development of educational materials for patients and staff including social media posts, the JPA newsletter, and patient-facing materials
• 10. Develop and execute patient education activities and events especially related to HIV awareness days and other health related topics
• 11. Prepare and present appropriate educational and other materials, using harm reduction approaches and evidence-based information both internally and at public events
• 12. Reduce barriers to obtaining clinical/care services by working with providers and other clinic staff to ensure that HIV care is treated as primary care
• 13. Maintain accurate and updated patient records, tracking contacts, counseling, testing, referrals, and outreach performance
• 14. Participate in staff meetings and staff development trainings
• 15. Perform special projects and other job-related duties as assigned

Minimum Qualifications:
