Job summary

ProHealth Physicians is seeking an Internal Medicine Registered Nurse to support the care team and patients in a community-based medical group. The role involves providing and documenting nursing care, triaging patients, and educating patients for self-management.

Qualifications

• BSN preferred
• 1+ years of nursing experience
• Active, unrestricted RN licensure in CT
• CPR/BLS Certification preferred

Responsibilities

• Provide and document patient care such as injections and preventive screenings.
• Triage patients according to approved protocols and scheduling guidelines.
• Prepare patients to be seen by collecting and recording clinical information as per protocol.
• Educate patients regarding their illness and self-management.
• Carry out standing orders including prescription refills and tests.
• Document all patient interactions and clinical activities in the EHR.
• Outreach to patients for appointments and test results.
• Answer and document patient calls and route to providers as needed.
• Monitor task buckets and referrals status.
