Qualifications

  • Residency training in an approved core specialty for board eligibility.
  • Proficiency in spoken and written English.

Responsibilities

  • Provide primary care within the VA PACT model and manage chronic conditions.
  • Lead multidisciplinary teams to optimize patient care and safety.
  • Document care in the electronic medical record and coordinate with specialists.
  • Participate in tele-health and patient education initiatives.
