Job summary

Colorado PA seeks a physician to join the Primary Care Service within the Eastern Colorado Health Care System. You will be part of a multidisciplinary Patient-Aligned Care Team (PACT) focused on comprehensive primary care for Veterans and families in the service area.

The role emphasizes performing histories and physicals, initiating therapies, and coordinating care across nursing, pharmacy, and other disciplines, with emphasis on chronic disease management and mental health integration.

Qualifications

• US citizenship required and VA policy adherence.
• MD or DO degree from an approved school.
• Current full and unrestricted medical license in the US.
• Residency training in an approved core specialty.
• Board certification or eligibility as applicable.
• Proficiency in spoken and written English.
• Pre-employment physical required.
• Ability to work within a PACT team-based care model.

Responsibilities

• Provide primary care services within the PACT team for Veterans and their significant others.
• Perform histories and physical examinations.
• Initiate and adjust therapeutic regimens.
• Order medications and supplies per VA formulary.
• Provide follow-up care and care coordination.
• Triage and refer to appropriate disciplines as needed.
• Collaborate with multidisciplinary team to plan care.
• Engage in health coaching, wellness, and chronic disease management.
• Support access to same-day face-to-face and telehealth visits.
