Qualifications

• The Primary Care Physician provides comprehensive primary care services to a panel of veterans and coordinates care within a multidisciplinary team.
• Emphasizes health maintenance, disease prevention, and chronic disease management.
• Participates in shared decision-making with veterans and families.
• Uses the PACT model to coordinate care among team members.
• Applies evidence-based practices to improve patient outcomes.
• Engages in quality improvement and educational activities within the team.

Responsibilities

• Deliver comprehensive primary care with emphasis on preventive health and chronic disease management.
• Coordinate care with specialty, mental health, and other services for veterans.
• Utilize VA’s electronic medical record system for documentation, orders, and communication.
• Support telehealth and video visits as part of primary care delivery.
• Provide mentorship and guidance to residents, students, and trainees.
