Qualifications

Responsibilities

• Provide operational oversight of the Care Management Department and lead care management programs.
• Standardize practices to reduce variation and improve metrics such as LOS.
• Drive quality, safety, and financial outcomes through interprofessional collaboration and strong communication.
• Be highly organized, patient-focused, and adaptable to changing needs; start within 2–3 weeks.
