Job summary

Five Keys Housing Services seeks a dedicated Care Coordinator to deliver comprehensive case management in interim housing settings. You will assess guests, craft individualized service plans, and connect them with housing, healthcare, and social supports to promote stability and progression to permanent housing.

The role requires strong collaboration with partner agencies, crisis intervention skills, and a commitment to trauma-informed care in a fast-paced environment.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling or related field.
• Minimum two years of care coordination or case management experience.
• Experience serving unhoused populations or in homeless services is preferred.

Responsibilities

• Conduct thorough guest needs assessments and document findings.
• Develop individualized care plans in partnership with guests and providers.
• Connect guests to housing, health care, social supports and employment resources.
• Advocate for guests and coordinate with shelters, agencies and community partners.
• Monitor progress, update plans, and provide follow-up aftercare support when needed.
