Job summary

Five Keys Housing is seeking a dedicated Care Coordinator to deliver comprehensive case management for guests in interim housing. You will assess needs, build individualized plans, and coordinate housing, medical, and social supports.

The role requires trauma‑informed practice, strong documentation, and collaboration with shelter staff and partner agencies to ensure smooth transitions to permanent housing and ongoing aftercare support.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or related field.
• Minimum two years of care coordination/case management experience.
• Minimum two years of paid work experience supporting unhoused populations with multiple disabilities.
• High school diploma or equivalent required.

Responsibilities

• Conduct thorough assessments of guests' needs, strengths, and challenges.
• Develop individualized care plans with guests and partners to meet housing, health, and social supports.
• Connect guests with housing programs, mental health services, substance use treatment, job training, and legal aid.
• Advocate for guests’ rights with shelters and community agencies; coordinate with service providers.
• Provide crisis intervention and follow-up support; document and monitor progress.
