Job summary

PAM Health is seeking a Case Manager to coordinate interdisciplinary care for an assigned patient population in a hospital setting. The role includes reviewing admission records to ensure appropriate resource utilization and determining level of care.

You will assess discharge needs, coordinate discharge planning, and work with the team to meet financial and quality goals while maintaining strong payor relationships. Licensure in the state and current BLS certification are required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines for designated patient populations.
• Review inpatient admission records to ensure proper resource use and admission level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.

Education
