Job summary

PAM Health is seeking a Case Manager to coordinate interdisciplinary care for an assigned patient population in a hospital setting. You will review admissions, assess discharge needs, and collaborate with the care team to ensure appropriate care levels and efficient resource use.

The role requires clinical licensure and 3–5 years of inpatient experience. Opportunity for growth, with a focus on quality outcomes and payor relationship management, in a supportive environment.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Experience: Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate care for designated patient populations across inpatient settings.
• Review admission records to ensure proper utilization and determine level of care.
• Identify discharge needs and coordinate discharge planning with the interdisciplinary team.
• Maintain and enhance payor relationships.
