Job summary

PAM Health Rehabilitation Hospital of Overland Park is seeking a Case Manager to coordinate interdisciplinary care for designated patient populations within an inpatient setting. The role involves reviewing admission records to ensure appropriate utilization of resources and determining admission levels of care.

The position requires working with the care team to plan discharges, coordinate discharge needs, and optimize financial and quality outcomes, while maintaining payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population.
• Performs reviews of inpatient admission records to ensure proper utilization of hospital resources and determination of admission for appropriate level of care.
• Assesses and identifies discharge needs and coordinates appropriate discharge plan.
• Works collaboratively with the interdisciplinary team to facilitate achievement of financial and quality outcomes.
• Maintains and enhances payor relationships.
