Job summary

PAM Health is seeking a Case Manager in Beaumont, TX to coordinate interdisciplinary care for a designated patient population. You will review inpatient records to ensure proper resource use and determine admission appropriateness, while identifying discharge needs and coordinating the discharge plan with the team.

You will work closely with nursing, therapists, and social work to meet quality and financial goals, and to strengthen payor relationships in a dynamic hospital setting.

Qualifications

• BLS certification required.
• Licensure in the state where the hospital resides is preferred. The designated case manager may include OT, PT, RN, RRT, SLP, or LMSW.

Responsibilities

• Coordinate care across interdisciplinary teams for a designated patient population.
• Review inpatient admission records to ensure appropriate level of care and resource utilization.
• Identify discharge needs and coordinate discharge plan with the team.
• Maintain and enhance payor relationships.
