Responsibilities

• Lead an 11-discipline Interdisciplinary Team (IDT) and coordinate care plans.
• Facilitate Care Team, Care Coordination, and Care Planning discussions.
• Ensure EMR documentation is clear, timely, and accurate.
• Develop and maintain compliant care planning workflows and regulatory alignment.
• Serve as an EMR superuser; train and assist team members.
