New Horizons of the Treasure Coast and Okeechobee is seeking an Intensive Case Manager to provide medically necessary, quality case management for adults with severe mental illness. You will engage clients, develop service plans, monitor progress, and assist with benefits access, within a supportive team.

The role requires a Bachelor's degree in a human services field, and CBHCM certification within 90 days, plus a valid driver's license and reliable vehicle.

Competenze

• Bachelor's degree in a human services field.
• Minimum of one year experience working with adults with serious mental illness.
• CBHCM certification required within 90 days of hire.
• Valid driver license with clean driving record and reliable vehicle.
• Must pass Level 2 Background check (DCF/AHCA Clearinghouse).

Mansioni

• Engage in staff hours of service for assigned individuals.
• Maximize revenue within ethical principles and good practice.
• Complete TCM assessments within the first 30 days of admission.
• Monitor client progress toward treatment goals across modalities.
• Coordinate individuals' care within and outside NHTC.
• Prepare written documentation for each service provided.
• Develop written service plans with input from individual and family.
• Coordinate referrals, admissions, discharges and transfers.
• Assist individuals to obtain benefits (SS, medical, food stamps, housing).
• Provide timely crisis intervention per program standards.
• Offer on-call emergency service after business hours, seven days.
• Provide transportation as needed following agency policies.
• Perform miscellaneous duties as instructed.
