Job summary

Benchmark Human Services is seeking an Intensive Support Coordinator to help individuals with intellectual and developmental disabilities live independently and participate in the community. You will coordinate services, advocate for needs, and maintain care plans with families, guardians, and interdisciplinary teams.

This role requires a bachelor’s degree in human services or a related field, a valid driver’s license, and willingness to travel up to 75%.

Qualifications

• Bachelor degree in human services, social work, sociology, or psychology.
• Must hold a valid driver’s license.
• Travel up to 75%.

Responsibilities

• Follow all company policies and HCBS rules.
• Perform evaluations and assessments to meet needs of individuals served.
• Assist individuals with applying for financial assistance, residential planning, vocational, recreational and educational services.
• Advocate for services supporting individual success.
• Maintain all case records and monitor Care Path Plans.
• Support person-centered plan development and coordinate meetings with families and guardians.
