Job summary

Benchmark Human Services is seeking an Intensive Support Coordinator to help individuals with intellectual and developmental disabilities live independently and participate in their communities. You will provide resources, support, and advocacy to maximize potential while following HCBS rules.

Qualifications

• Bachelor degree in human services, social work, sociology, or psychology.
• Valid driver’s license and auto insurance.
• Minimum 2 years in a similar position.
• Travel up to 75%.

Responsibilities

• Follow policies and HCBS rules.
• Perform evaluations and assessments for individuals served.
• Assist with applying for financial assistance, planning, healthcare, and services.
• Advocate for services to support the individual’s success.
• Maintain all case records and prepare Care Path Plans.
• Provide support to participate in person-centered planning.
• Coordinate meetings with individuals, families, guardians, and the team.
• Record work and billing time per company policies.
• Track, monitor, and enter specified data points.
• Report any suspected abuse, neglect, or exploitation.
