Job summary

Benchmark Human Services in Georgia is seeking an Intensive Support Coordinator to help individuals with intellectual and developmental disabilities live more independently and participate in their community. You will perform evaluations, coordinate services, advocate for clients, and maintain case records.

Qualifications

• Bachelor degree in human services, social work, sociology, psychology.
• Must hold a valid driver’s license.
• Maintain auto insurance.
• Minimum of 2 years working in a similar position.
• Travel up to 75%.

Responsibilities

• Follow company, Georgia Department of Community Health and HCBS rules.
• Perform evaluations and assessments to meet individuals’ needs.
• Assist individuals with applying for financial assistance, residential planning, vocational, recreational and educational desires, healthcare, in-home supports, etc.
