Job summary

Benchmark Human Services is seeking an Intensive Support Coordinator to help individuals with disabilities achieve independence and community involvement.

You will conduct evaluations, coordinate services, advocate for resources, and maintain case records, working with families and guardians under HCBS guidelines. A bachelor’s degree in a related field and a valid driver’s license are required; travel up to 75% is expected.

Qualifications

• Bachelor's degree in human services, social work, sociology, or psychology.
• Valid driver's license.
• Auto insurance.
• Minimum of 2 years in a similar position.
• Travel up to 75%.

Responsibilities

• Follow company policies and HCBS rules.
• Perform evaluations and assessments.
• Assist individuals with applying for financial assistance and services.
• Advocate for services to support client success.
• Maintain all case records and care plans.
• Coordinate meetings with families and guardians.
